The information containted in this application is correct to the best of my knowledge. Should this application be accepted, my child agrees to be bound by any policies/procedures of Queensland Baptists and Hervey Bay Baptist Church and refrain from unscriptural conduct in the performance of their services on behalf of the church.

My child and I have been provided a copy of and agree to abide by the Code of Conduct.

I understand the following:

- by signing this form, my child is committing to the voluntary position that will be made by mutual agreement between the relevant Hervey Bay Baptist Church Ministry Leader and me. This position can be reviewed or terminated at any time by the Hervey Bay Baptist Church leadership team with adequate and appropriate notification between all parties, or by me with adequate notification to the ministry leader.

- if my child is granted access to the church database (Elvanto), they are to use this strictly for the purposes of performing their role and am not to access the information contained therein inappropriately or for their own personal gain. Failure to abide by this may result in having their database permissions revoked, and/or being stood down from their voluntary position without notice, or where appropriate, legal action may be taken.
