Car Travel

For the purpose of attending an offsite HBBC activity, do you give permission for your child to travel in a car driven by an approved licensed driver from HBBC?

Correspondence

Your details will be used to contact you regarding any changes to the program you have registered your child/ren for.  In some instances, leaders of programs may wish to contact you and/or your child/ren via social media (where applicable) for communication of program information (eg. group chats).  We value and respect the privacy and integrity of our U18 participants and will never contact your child/ren via social media without your prior consent and knowledge.

Do you give permission for a HBBC leader to contact you &/or your child/ren for follow-up support or to receive information regarding relevant events for families?

Bus Travel

For the purpose of attending an offsite HBBC activity, do you give permission for your child to travel in the Hervey Bay Baptist Church Bus/People Mover(approved drivers only)?

External Promotions

Registration in our programs provides HBBC consent to take photographs or video footage of the above-named child/ren for use in internal promotions.

Do you give permission for HBBC to use and publish the images for external promotions?

Activities

Our regular events and programs offer a range of general sporting and recreational activities which may include- Water activities; casual games and activities; organized games and activities; sleepovers; bush walking; campfires; cooking.

Specific parental consent will be sought for other activities e.g. Camps etc.

Code of Behaviour

Hervey Bay Baptist Church (HBBC) is committed to providing a safe and positive environment where young people have heaps of fun and learn and grow in relationship with God and each other. The following code of behaviour is necessary to maintain a safe environment both within our programs and while a passenger on our church bus.

•All participants of HBBC U18 events are required to register on their arrival at every event.

•Throughout the event, participants must stay within the designated areas and be in view of a leader or adult at all times.

•Participants must follow the direction of the event leaders at all times.

•Prohibited items at our events: alcohol, cigarettes, illicit drugs, knives or weapons.

•Unwanted or excessive physical contact between young people during events is prohibited.

•Any act that endangers any participant or leader is prohibited.

•Bullying is unacceptable.

•Swearing and offensive language are not permitted.

During an event, everyone has a right to participate without distraction, and leaders have a right to communicate without distraction. Anything that inhibits this is unacceptable, (talking, physically distracting).

•All mobile phones should be silenced or turned off during program times.

•No expensive personal items (jewellery or the like) are to be brought to our programs.

•Respect and care should be shown for other people’s belongings and the equipment that is used.

•No participant will be permitted to leave an event early without parent/guardian permission.

•As our events are mixed gender events, appropriate clothing is required. Footwear must be worn at events at all times, unless specifically instructed by the leaders otherwise.

•Where a transport vehicle is used (ie. people mover/bus), all passengers in the vehicle should remain seated whilst the vehicle is moving and keep all limbs inside the vehicle at all times. Driver’s instructions must be followed at all times. No food, drinks or chewing gum are permitted while in the vehicle.  

Failure to abide with the above will result in a caution, suspension or indefinite ban depending on the severity of the breach.  The choice of disciplinary action will be at the complete discretion of the activity leader in consultation with the leadership of HBBC.  The parent/guardian will be informed of the details of the incident and the outcome.

The Hervey Bay Baptist church is committed to the safety and wellbeing of all children involved in our programs. We support the rights of the child and will act without hesitation to ensure a safe child environment is maintained at all times.

We also support the rights and wellbeing of our staff and volunteers and encourage their active participation in building and maintaining a secure and safe environment for all participants.

Key Information

In order to provide the highest level of care for the children under 18 in our programs, HBBC has implemented a Risk Management Strategy. Our Risk Management policy and procedures are available on request.

In terms of our Policy, our leaders hold current Blue Cards, as required by law, but also are given input and ongoing training in how to effectively and appropriately work with children under 18. We expect all our leaders to display healthy Christian principles and values in their leadership and relationships.

Parents / guardians are welcome to observe the operations of our children and youth programs.

In the case of a medical emergency the leaders will make all practical attempts to contact you or the emergency contact person listed. Where this is not possible, the leader will arrange for the minor to receive treatment, if in his / her judgement it is deemed necessary.

It is your responsibility to ensure that the information supplied on this enrolment form remains current at all times. Any changes need to be in writing and forwarded to the Church office.

Privacy

The Australian Privacy Principles (APPs) set out in the Privacy Act 1988 (Cwlth) apply to Hervey Bay Baptist Church.

We collect personal information from you for the purpose of maintaining our records/database about members and affiliated people so we can provide services to you.  If incomplete or inaccurate information is provided, we may not be able to provide efficient services to you.

The Privacy Act allows you to have access to and correct records of personal information held by us.  If you wish to make an inquiry or complaint regarding the information we hold about you, you can contact our Ministry Facilitator who is:

Your Agreement with Hervey Bay Baptist Church (HBBC)

I have read through the Code of Behaviour with my child/children and we agree to support the HBBC leadership team in upholding the HBBC Code of Behaviour.

I am aware, in signing this document regarding my child’s participation in this program, that certain elements of the program could be physically and emotionally demanding.  Furthermore, I understand that certain inherent risks and dangers may exist in the activities in which my child will be participating.  I acknowledge that while the organisation and its leaders will make every reasonable effort to minimise exposure to known risks, all hazards and dangers associated with these activities cannot be foreseen or may be beyond the control of the Hervey Bay Baptist Church, its leaders and staff. 

In the event of any emergency, where my nominated contact people are unavailable:

  • I authorise the leaders to obtain medical advice and/or assistance which they deem necessary.
  • I accept the responsibility for payment and agree to pay medical, transport and any other related expenses.
